Outpatient RN Care Manager, Ventilator Assisted Care Program

Essential Functions The RN Care Manager facilitates and coordinates the patient's treatment plan of care to assure quality through the continuum of care. Collaborates with the interdisciplinary team to implement the plan of care and transition strategies, ensuring the achievement of desirable patient outcomes, appropriate length of stay, efficient utilization of resources, increased patient and family involvement, and patient/family/staff education. Experience Qualifications 2 years of professional nursing or care management experience. Licenses and Certifications Basic Life Support Health Care Provider (CPR, BLS) - American Heart Association (required for those employees working on site at LCMC Health Facilities) Registered Nursing License (RN) - Louisiana State Board Of Nursing BSN Required Skills and Abilities Ability to work successfully in a stressful environment, relate well to all individuals, and maintain expanding knowledge and use of clinic and hospital information systems and population health/call center platforms. Must have strong clinical judgement and a broad knowledge base across a wide spectrum of medical conditions. Must be proficient with Windows-style applications and keyboard. Must have excellent organizational and time management skills. Basic Life Support Health Care Provider (CPR, BLS) - American Heart Association and written communication skills and the ability to present information clearly and professionally to varying levels of individuals.
