Job Description
Meadow Park Rehabilitation & Healthcare Center is seeking a dedicated and compassionate Full-Time Physical Therapist to join our rehabilitation team. This is an excellent opportunity for a motivated clinician to make a meaningful impact while working in a collaborative and resident-focused environment.
ResponsibilitiesEvaluate residents and develop individualized treatment plans.
Provide skilled physical therapy services to short-term rehabilitation and long-term care residents.
Monitor resident progress and modify treatment plans as appropriate.
Collaborate with physicians, nursing staff, and interdisciplinary team members.
Maintain timely and accurate documentation in accordance with facility and regulatory requirements.
Educate residents and families regarding treatment goals and therapeutic programs.
Current New York State Physical Therapist license in good standing.
Strong clinical, communication, and organizational skills.
Skilled nursing facility experience preferred, but not required.
