Meadow Park Rehabilitation & Healthcare Center is seeking a dedicated and compassionate Full-Time Physical Therapist to join our rehabilitation team. This is an excellent opportunity for a motivated clinician to make a meaningful impact while working in a collaborative and resident-focused environment.

Responsibilities

• Evaluate residents and develop individualized treatment plans.

• Provide skilled physical therapy services to short-term rehabilitation and long-term care residents.

• Monitor resident progress and modify treatment plans as appropriate.

• Collaborate with physicians, nursing staff, and interdisciplinary team members.

• Maintain timely and accurate documentation in accordance with facility and regulatory requirements.

• Educate residents and families regarding treatment goals and therapeutic programs.

Qualifications

• Current New York State Physical Therapist license in good standing.

• Strong clinical, communication, and organizational skills.

• Skilled nursing facility experience preferred, but not required.
