Hospice Admissions Nurse: Lead Patient & Family Care

Advocate Health is seeking a part-time Hospice Patient Access Nurse to admit patients to hospice services and lead the interdisciplinary care team on their caseload. The role requires an active RN license in the state, an Associate's Degree in Nursing, and at least two years of direct patient care.
