RN Care Manager - LTSS & Care Coordination

Molina Healthcare

Molina Healthcare seeks an RN care manager to support LTSS and care coordination for high-need members. You will collaborate with a multidisciplinary team to ensure quality and cost-effective member care, including home visits and waiver-related activities.

You will assess needs, develop care plans, monitor outcomes, and coordinate services across behavioral health and LTSS, while navigating regulations and funding sources. Strong communication and independent work are essential.
