Position Overview

We are seeking a skilled and compassionate Outpatient Physical Therapist for a 13-week contract assignment. This opportunity is ideal for a Physical Therapist who enjoys providing individualized patient care and working collaboratively with a clinical team.

The Physical Therapist will evaluate patients, develop personalized treatment plans, provide evidence-based interventions, and help patients improve function and safely return to their daily and community activities.

Key Responsibilities

Conduct comprehensive physical therapy evaluations for assigned patients.

Develop individualized treatment plans based on patient needs, goals, medical history, and physician direction.

Provide 60-minute evaluation and treatment sessions.

Implement, modify, and progress treatment plans based on patient response and clinical needs.

Develop functional treatment programs designed to improve mobility, independence, and quality of life.

Educate patients and families on treatment plans, therapeutic exercises, home programs, and injury prevention.

Monitor and document patient progress accurately and efficiently.

Maintain complete and timely medical records in accordance with applicable regulations and clinic policies.

Provide care in accordance with valid physician orders and applicable insurance authorizations.

Collaborate with other healthcare professionals to coordinate patient care.

Participate in quality improvement initiatives.

Maintain professional licensure and required continuing education.

Follow applicable state practice acts, professional standards, and ethical guidelines.

Promote a safe environment for patients, coworkers, and visitors.

Qualifications

Degree in Physical Therapy from an accredited program.

Active Physical Therapy license in the state where services will be provided.

1+ year of outpatient clinical experience required.

Current CPR certification required.

Strong clinical reasoning, communication, and organizational skills.

Ability to develop and implement individualized treatment plans.

Commitment to patient-centered care and positive clinical outcomes.

Ability to work effectively in a collaborative healthcare environment.
