Overview
Licensed Practical Nurse, Long Term Care, Pittsfield, MA starting 08/31/2026 for 13 weeks, 40 hours per week, days shift. Weekly pay: $1,804 to $1,947. Travel - temporary assignment in a Home Healthcare - Hospice setting. No guaranteed hours.

Responsibilities
- Provide and document skilled nursing care to patients in their homes per the plan of care.
- Administer medications and therapies; monitor patient status and report changes; intervene under approved protocols.
- Collaborate with the care team, including physicians, therapists, and family members, to coordinate comprehensive home-based care.
- Evaluate patient and family responses to interventions; adjust care plans as needed; maintain accurate and timely documentation.
- Demonstrate independent clinical judgment and adhere to policies, protocols, and confidentiality requirements; manage time effectively and respond to emergencies as needed.

Requirements
- Active Massachusetts LPN license (Massachusetts Temporary Nursing Service Agency License/Nursing Pool) must be uploaded with the traveler profile.
- BLS certification (American Heart Association or American Red Cross) required.
- Minimum 1 year of acute nursing care experience; hospital-based experience preferred.
- Home health experience required; proficiency with OASIS highly preferred; infusion therapy experience preferred.
- Valid driver’s license and access to a personal vehicle; traveler must provide own vehicle; ability to drive in inclement weather.
- Flexible with work hours; willing to accept on-call responsibilities; RTO requests must be included in profile and are limited to no more than 5 days.
