Physical Therapist – Outpatient

Schedule: Day Shift M - F, 8:30am to 5:00pm

Assignment Details:

• Contract Length: 13 weeks
• Guaranteed Hours: Not specified

Requirements:

• Physical Therapy licensure in the state of practice: Required
• BLS Certification: Required
• 1 year experience required; prefers 2 years experience
• Must have worked in the submitted unit specialty within the last 3 months

Roles & Responsibilities:

• Evaluate and assess patient needs for physical therapy interventions.
• Develop and implement individualized treatment plans based on patient goals and objectives.
• Administer therapeutic exercises and modalities to enhance patient recovery.
• Monitor patient progress and adjust treatment as necessary.
• Educate patients and families on treatment plans and home exercise programs.
• Collaborate with healthcare team members to provide comprehensive care.

Compliance / Notes:

• Make sure to include RTO, Birthdate, and Social.
• Admin fee is already deducted from rate.
• No Call required for this role.
