Occupational Therapist

Job Overview A compassionate and skilled Occupational Therapist is needed to join an interdisciplinary team (IDT) focused on providing high-quality, patient-centered care . This role is essential in coordinating and delivering rehabilitative occupational therapy services to support participants in achieving maximum independence and function , both at the center and in-home settings . Key Responsibilities Conduct comprehensive Occupational Therapy assessments to determine patient needs, preferences, and functional goals . Develop and implement customized therapy plans in collaboration with the IDT. Deliver and document therapeutic interventions , including: Use of devices, techniques, and activities to promote muscle re-education, cognitive function, balance, endurance, and ADLs (Activities of Daily Living) . Supervise Therapy Assistants , offering training and guidance for Functional Maintenance Programs (FMP) . Provide education and training to staff, caregivers, and families on occupational therapy techniques. Work with the IDT to ensure smooth transitions between care settings (e.g., hospitals, skilled nursing facilities, and home care ). Participate in end-of-life care coordination and support when needed. Qualifications Bachelor’s Degree in Occupational Therapy from an AOTA & ACOTE accredited program . Minimum of 2 years of occupational therapy experience in a clinical setting (preferably with frail or elderly populations). Supervisory experience (preferred) – ability to mentor and develop team members. National Board Certification in Occupational Therapy (NBCOT) . Valid California Occupational Therapy License (or eligibility for licensure).
