Requirements:

• Active and valid Texas state license
• 3-5 years of clinical experience
• Strong communication and interpersonal skills
• Ability to include your viewpoint in the care of patients
• Reliable transportation for home-based visits
• Passion for providing high-quality, patient-centered care
