Loudoun Home Care is seeking a compassionate and dependable Registered Nurse (RN) to join our growing home care team. The RN will provide skilled nursing care to clients in their homes, coordinate care, and ensure services are delivered safely and in accordance with the individualized plan of care, physician orders, agency policies, and applicable Virginia regulations.

General Accountabilities

• Perform comprehensive nursing assessments and participate in client admissions, care planning, and discharge planning.
• Develop, implement, and regularly update individualized plans of care.
• Provide skilled nursing services, including medication administration, treatments, wound care, and other services within the RN's scope of practice.
• Monitor clients' conditions and promptly report significant changes to physicians, the Director of Nursing, and other appropriate members of the care team.
• Document assessments, interventions, progress, and other nursing services accurately and in a timely manner.
• Supervise and provide guidance to LPNs, home health aides, and other assigned staff as appropriate.
• Conduct required supervisory and reassessment visits.
• Educate clients, families, and caregivers regarding medications, disease management, treatments, safety, and care needs.
• Coordinate care with physicians, families, caregivers, and other healthcare professionals.
• Promote infection prevention, client safety, and quality of care.
• Participate in quality improvement activities, staff education, and agency training initiatives.
• Maintain compliance with Virginia regulations, applicable standards, scope of practice, and agency policies.

Job Qualifications

• Current, unrestricted Virginia Registered Nurse (RN) license or valid Compact RN license.
• Minimum 1 year of nursing experience; home health, home care, community health, or private duty nursing experience preferred.
• Strong assessment, clinical judgment, communication, and documentation skills.
• Ability to work independently and confidently in a client's home.
• Dependable, compassionate, and professional approach to client care.
• Ability to communicate effectively with clients, families, caregivers, physicians, and other healthcare professionals.
• Knowledge of home care regulations and nursing standards preferred.
• Current CPR/BLS certification preferred or required within agency guidelines.
• Experienced with G-tube and Trach.
