What You’ll Do

The Transitional Housing Program located North of the Panhandle serves 20 families for 12-18 months. Families participating in our residential programs receive comprehensive social services to identify and address factors contributing to their homelessness to enable them to return to stable, independent housing and economic self-sufficiency, including: case management with referrals and linkages to community services and benefits, employment and life skills training, access to basic health services, children’s services, and innovative and direct assistance in securing permanent housing.

The PT temporary Case Manager provides a limited number of families with case management and support to assist them with gaining stability and obtaining permanent housing, while we onboard new staff. The purpose of this role is to ensure continuity of care with a select number of families.

What Your Day Looks Like

• Provide a select number of residents with case management services with a focus on exit planning and family success once the family exits THP services.
• Develop, implement and document family action exit plans with participants that identify and address their needs including but not limited to housing options, recovery issues, children’s needs, and short-term goals. Identify, network with, and refer participants to community-based services as appropriate.
• Communicate and collaborate with other case managers, supervisors, program staff, and community partners and service providers to provide optimal support to families.
• Follow and reinforce all program policies, rules and expectations for families to participate in the program.
• Complete and maintain required handwritten and computer-based records, files, correspondence, and statistics in a timely, clear and thorough, accurate, and legible manner. Compile, complete and submit required reports and information documenting services provided to participants, and data related to program service and outcome objectives. Review and present progress of each family in regular case conferences. Prepare reports and presentations as required.

• Highly organized, self-motivated, and capable of working independently to meet goals

• Demonstrated ability to exercise appropriate judgement and uphold policies and procedures

• Skilled in documentation, reporting, and maintaining confidentiality

What You Bring

• A minimum of 2 years of experience providing support services to families and/or individuals in a human services setting, OR a bachelor’s degree in social work, psychology, or related field from an accredited college or university and a minimum of one year of experience providing support services to families and/or individuals in a human services setting.

• High School Diploma or GED required.

• Experience working in Case Management, preferred.

• Experience working in a residential facility, preferred.

• E working with and sensitive to the needs of homeless families.

• Able and willing to work with diverse staff and participants.

• Experience in a crisis setting and able to respond appropriately to emergencies including contacting appropriate staff and/or contacting and interacting with police, fire, and medical personnel as needed.

• Experience providing support services to participants with mental health related issues, substance abuse, domestic violence, HIV/AIDS related issues, Child Protective Service agencies, HIPAA and crisis intervention techniques.

• Excellent written and verbal communication skills; proficient in Microsoft Office (Word, PowerPoint, Outlook, Excel, etc.). Able to make regular entries into a web-based participant database.

• Must be available for occasional evening hours as work schedule includes some evenings.

• Essential functions of the position may require use of a telephone, working at a standard computer terminal, and the ability to follow reasonable ergonomic recommendations, sit for long periods, stand, reach, bend, lift, and carry up to 40 pounds.

• CPR and First Aid certification required within first 6 months of hire.

• Criminal background check and fingerprint imaging required post offer.

• TB (Tuberculosis) clearance and documentation required post-offer.
