Job Title: Registered Nurse (RN) Case Manager – Home Health, 1:1 Care & Autonomy

Company: Home Health Care, Inc.

Location: Greater Twin Cities Metro Area (21-county service area)

Schedule: Full-Time, Monday to Friday, 8:00 AM – 5:00 PM, 1 weekend per month, 1 on-call shift per week

Compensation: $85,000 – $130,000 annually plus $6,000 in first-year bonuses

What You’ll Do

• Complete comprehensive nursing assessments and care plans .

• Perform OASIS visits and coordinate care across settings .

• Provide skilled nursing care, education, and support to patients and families.

• Collaborate with physicians, therapists, and interdisciplinary teams.

• Manage a caseload of patients in homes, assisted living, and community settings .

• Maintain accurate, timely documentation and regulatory compliance .

What We’re Looking For

• Active Minnesota Registered Nurse (RN) license.

• 3+ years of RN experience, home health preferred.

• Strong assessment, care planning, and documentation skills .

• Ability to work independently in a field-based role .

• Reliable transportation and valid driver’s license.
