Outpatient Physical Therapist

Job Description

We are seeking a dedicated Outpatient Physical Therapist to evaluate assigned patients and develop, implement, and modify individualized treatment plans. The ideal candidate will adhere to patient goals and physician directions, ensuring optimal rehabilitation outcomes. This role encompasses educating patients and families, supervising support staff, and maintaining compliance with professional standards and organizational values.

Responsibilities

• Conduct comprehensive patient evaluations and formulate appropriate treatment plans.
• Document patient progress and treatment effectiveness in compliance with state and federal regulations.
• Ensure all care is provided under valid physician orders and insurance approvals.
• Supervise students and contribute to their education and development.
• Engage in professional development and continuous education activities.
• Adhere to safety protocols and promote a safe environment for patients and staff.
• Participate in performance improvement initiatives to enhance service quality.

Schedule

This is a 5x8.0 shift position, starting immediately.

Required Experience / Certifications / Licensure

• Degree in Physical Therapy from an accredited institution.
• Current, valid licensure to practice as a Physical Therapist.
• CPR certification.
• Minimum of 6 months to 1 year of outpatient clinical experience preferred.
