In-Home Nurse Practitioner or Physician Assistant

Signify Health seeks a part-time In-Home Nurse Practitioner or Physician Assistant to perform comprehensive health evaluations in patients’ homes. You will review medical histories, conduct physical exams, assess social determinants of health, reconcile medications, and document findings in our technology platform to support value-based care. You’ll collaborate virtually with interdisciplinary teams to close care gaps, guide care plans, and connect vulnerable patients to appropriate community and clinical resources, while benefiting from flexible scheduling and a supportive, data-driven culture focused on outcomes.
