This role includes routine skilled nursing visits and may include assessment‑related visits under RN direction , with compensation structured per visit or hourly depending on the case and service provided . This position is ideal for LPNs who are confident working independently, experienced in home health, and understand the importance of accurate, detailed clinical documentation in supporting patient care and nursing oversight. Flexible Compensation Structure Compensation may be per visit or hourly , depending on the case Assessment‑related visits and routine visits are compensated differently based on scope and responsibility Flexible Scheduling PRN, per‑diem, or flexible caseloads Visit schedules coordinated based on nurse availability and patient needs Key Responsibilities Perform routine skilled nursing visits as ordered Assist with patient assessments under RN direction, as applicable Administer medications and treatments within LPN scope of practice Provide wound care, catheter care, and other skilled interventions Monitor patient condition and report changes to the supervising RN Educate patients and caregivers on care plans and treatment instructions Complete accurate, timely, and defensible clinical documentation Documentation Expectations LPN documentation is a critical part of patient care and is used to: Support RN oversight and clinical decision‑making Contribute to the development and revision of the patient’s Plan of Care Meet payer and regulatory documentation requirements Strong attention to detail and timely completion of visit notes is required.
