Exciting Opportunity for RN Case Manager Job Description: - Contract Length: 13 Weeks - Shift: Day Shift, 8:00 AM - 4:30 PM - No Call Required - Work Schedule: 5 eight-hour shifts per week, rotating weekends Unit Information: - Location: Med Plaza 1, 4th Floor, Suite 411 - Staffing: 22 dedicated employees including Case Managers, Social Workers, and Case Management Assistants - Patient Ratios: 25:1 for Case Managers, 40:1 for Social Workers - Equipment Used: MIDAS, CAREPORT, INTERQUAL - Electronic Medical Record (EMR): MEDITECH - Permanent Positions Available: Yes, 3.5 FTEs Typical Responsibilities: - Case Manager: - Provide case management services for both inpatient and observation patients - Conduct comprehensive assessments of psychosocial, medical, and discharge needs - Coordinate the plan of care and actively drive the discharge process in collaboration with a multidisciplinary team - Evaluate progression of care using InterQual standards - Social Worker: - Collaborate with RN case managers and the multidisciplinary team - Perform assessments and craft effective discharge plans for high-risk patients - Engage in consultation, education, counseling, advocacy, and referrals to address emotional, social, and financial aspects of care Ideal Candidate Traits: - Calm under pressure and able to communicate clear and concise information - Trustworthy and authentic in interactions - Strong multitasking and prioritization skills - Flexible and adaptable to change - Team player with a positive attitude Dress Code: - Business casual attire Unit Achievements: - Consistent Length of Stay Ratio Catalytic Solutions Job ID 19012114.
