APRN: Remote Patient Monitoring & Home Visits Specialist

Dartmouth-Hitchcock Medical Center

Dartmouth-Hitchcock Medical Center is seeking an innovative Advanced Practice Provider for the Remote Patient Monitoring program. This role involves meeting discharged adult patients at their homes for assessments and equipment setup, ensuring seamless care between inpatient and outpatient settings.

The ideal candidate must have graduated from an accredited APRN program, hold board certification, and be eligible for licensure in New Hampshire. Responsibilities include conducting home visits, collaborating with healthcare teams, and providing patient education.
