Job Overview

Visiting Nurse Health System is hiring a Admissions Nurse for Hospice, responsible for coordinating care for a caseload of patients and ensuring it meets quality standards and utilization guidelines. The RN assesses, plans, implements, monitors, and evaluates comprehensive care plans to ensure optimal patient outcomes. This position demands a compassionate, skilled nurse who thrives in a home-based environment and values patient education and family engagement. This role will service Cobb, Douglas, Fulton and surrounding areas.

What You’ll Do:

• Develop and implement individualized care plans in accordance with physician orders by assessing patient needs and collaborating with the interdisciplinary team.
• Modify and update care plans as patient conditions change, regularly evaluating their effectiveness.
• Provide patient and family education on disease processes, self-management, and strategies to prevent complications such as hospitalizations.
• Complete and submit OASIS documents, visit notes, and other required records within VNHS timeliness guidelines.
• Communicate changes in patient status clearly with patients, physicians, and team members, documenting appropriately.
• Offer emotional support and connect patients/families with MSW and other community resources as needed.
• Supervise LPNs and HHAs per VNHS policy, ensuring care plans are in place and meet patient needs.
• Participate in SOC/ROC conference calls for new or resuming patients and in weekly team conferences.
• Deliver competent, high-quality care in the home, adhering to VNHS policies and nursing standards.
• Maintain required competencies through in-services and continuing education.
• Schedule patient visits efficiently in accordance with orders and manage case load to optimize productivity.
• Precept newly hired nursing staff and communicate effectively with the team and office staff regarding patient status or concerns.
• Assesses and admits patients for admission to the Hospice Atlanta Program. Admits patients for care based on skilled need and payor source eligibility requirements.
• Participates in the on-call rotation, demonstrates flexibility by seeing patients outside of their regular schedule as needed, and meets patient care needs in accordance with program requirements.

• Graduated from an accredited nursing program (Diploma, Associate, or BSN preferred).
• Hold current licensure by the Georgia State Board of Nursing and valid CPR certification.
• Possess a valid driver’s license, reliable transportation, and current auto insurance.
