North Kansas City Hospital Home Health, LLC

North Kansas City Hospital Home Health seeks an experienced Registered Nurse Case Manager to lead patient assessment and plan of care in the home setting. You will coordinate multidisciplinary care, educate patients and families, and ensure documentation meets Medicare guidelines while staying compliant with state laws.
