Licensed Practical Nurse (LPN)

Provides, coordinates, and directs the provision of home nursing care according to physician orders. Care is based on agency policies and procedures through the competent application of the nursing process. Responsible to the Clinical Manager. Directly supervises and evaluates Home Health Aides and Licensed Practical Nurses. Degree of travel involves home visits and periodic office meetings. Must have reliable transportation, a current driver's license, and agency-required liability insurance. Average number of patients per day is 5-8. Each type of visit and activity holds a different point value, with 30 points expected per week. Contractor will be assigned between 5-10 patients per day, equating to roughly 6-8 points. If unable to meet financial needs due to low patient census, please speak with the Account Manager about floating to a nearby clinic to pick up hours. Types of cases include Start of Care, Wound Care, and Follow-up Visits. First-timers are not accepted; recent specialty experience is required. Weekend requirements vary by location and profession. On-call requirements also vary. Must be willing to travel up to 75 miles from the home clinic. Mileage will be reimbursed at the IRS standard rate. Local candidates are accepted at the same rate. Pending licenses are not accepted; candidates must have a valid license in hand. Travelers should be open to various scheduling needs. Dress code requires scrubs. All personnel must be proficient with HomeCare HomeBase (HCHB) and have used it within the last 6 months. Those who have used it beyond 6 months may apply but will need to complete retraining. The contractor must accept every patient assigned regardless of the distance between appointments. All documentation must be completed within 24 hours of the patient appointment. Delayed and incomplete documentation can impact billing and patient care follow-up. There are no guaranteed hours for this contract, and the clinician must achieve weekly productivity points of 30. All overtime must be pre-approved.
