Aveanna Healthcare Private Duty Nurse LVN:Full Time/Part Time Weekend Nights

Position Overview The Licensed Practical Nurse (LPN/LVN) is responsible for providing and documenting skilled nursing care, under the supervision of a Registered Nurse, in accordance with the developed care plan and physician's orders for each individual patient while adhering to confidentiality standards and professional boundaries at all times. Location San Antonio, TX 78251, 78242, 78245, 78207 and surrounding areas Key Responsibilities Delivery and coordination of quality patient care in compliance with physician orders. Continuously observe and assess patient condition and care needs and report changes to supervisor and/or physician. Document all activities, assessments, nursing actions and care coordination in a timely manner. Participate in implementation and updating of the nursing care plan. Take appropriate nursing action based on assessment and achieve expected outcomes. Recognize changes in patient needs and intervene to prevent or reduce risk. Accept responsibility for personal and professional accountability by complying with company policies, state and federal regulations, accrediting bodies and the Nurse Practice Act. Provide care using infection control measures that protect staff and patients under OSHA standards. Educate patient and family about disease process, self‑care techniques, and prevention strategies. Maintain knowledge of nursing competencies through educational programs, continued education units, and skill evaluations. Qualifications Graduate of an accredited nursing school. Current, unrestricted state license as an LPN/LVN in the state of practice. Current CPR certification with hands‑on component. Demonstrated proficiency in clinical assessment, documentation and compliance with nursing care policies and procedures. State‑specific requirements: South Carolina – one year of pediatrics experience. California – one year of experience working under a current nursing license. Louisiana – one year of experience working as a licensed nurse. Continuing education as required by state. Preferences Six months of recent experience as a Licensed Nurse in a clinical setting. Home‑health experience. Other Skills / Abilities Attention to detail. Time management. Effective problem‑solving and conflict resolution. Good organizational and communication skills. Physical Requirements Ability to speak, read, write and understand English. Ability to travel. Ability to lift 50 pounds. Ability to reposition patients and move equipment without assistance. Prolonged walking, standing, bending, kneeling, reaching, twisting. Ability to sit and climb stairs. Visual and hearing acuity. Strong sense of smell and touch. Ability to respond physically and mentally to emergency situations in the home or during transport. Environment Function in a variety of environments that may involve exposure to allergens. Possible exposure to blood, bodily fluids and infectious diseases.
