Key Responsibilities\n- \n
- Conduct thorough psychosocial assessments to identify clients’ emotional, psychological, and social needs relevant to hospice care. \n

- Develop and maintain comprehensive individualized treatment plans incorporating client goals, interventions, and progress monitoring. \n
- Collaborate effectively with interdisciplinary teams, including physicians and nurses, to provide coordinated, holistic care for clients. \n
- Offer crisis intervention and support as required, including risk assessments and safety planning for vulnerable clients. \n
- Facilitate support groups and educational sessions, focusing on coping strategies for clients and their families in hospice situations. \n
- Ensure meticulous and timely documentation in compliance with ethical and legal standards, utilizing digital documentation systems for efficiency. \n
Your work will be focused on supporting clients and families who are navigating the complexities of hospice care, addressing issues such as grief, anxiety, and adjustment to end-of-life transitions. Qualifications\n- \n
- A Master's degree in Social Work from an accredited institution. \n
- Active state licensure as a Licensed Clinical Social Worker (LCSW). \n
- Minimum of two years of relevant experience in mental health or hospice care settings. \n
- Solid understanding of evidence-based therapeutic modalities including but not limited to cognitive-behavioral therapy and solution-focused brief therapy. \n
- Exceptional skills in interpersonal communication, assessment, and crisis management. \n
