Job Description:

JOB TITLE: PRN Retail Pharmacist

JOB SUMMARY: The retail pharmacist is responsible for all facets of the medication order verification, dispensing and documentation process for all prescriptions received for processing in the retail pharmacy. The pharmacist will be actively involved in the provision of pharmaceutical care including: assisting with patient interviews, maintenance of current medication lists, drug utilization reviews on all prescriptions, providing patient medication education, providing drug information to the healthcare team and patients, and counseling of patients for all new prescriptions, refills, and/or over-the-counter medications as necessary. The pharmacist will practice with a high degree of autonomy, guided by policy and procedures and quality assurance verification guidelines. The pharmacist is also responsible for the daily support of all retail pharmacy staff.

DUTIES AND RESPONSIBILITIES:

Responsible for accuracy of all dispensed prescriptions. Pharmacist will verify all prescription information, perform patient drug regimen and utilization review, insure an absence of known drug interactions, as well as counsel all patients.

Counseling is defined as: Communication to the patient or the patient's agent regarding information about the prescription drug or device which, in the exercise of the pharmacist's professional judgment, the pharmacist deems significant, such as the following: the name and description of the drug or device; dosage form, dose, route of administration, and duration of drug therapy; any special directions and/or precautions for preparation, administration, and use by the patient; common severe side/adverse effects, interactions, and therapeutic contraindications that may be encountered, including their avoidance, and the action required if they occur; techniques for self-monitoring of drug therapy; proper storage; refill information; and action to be taken in the event of a missed dose.

Counseling shall be provided with each new prescription drug order and/or offered for all prescription drug order dispensed by the pharmacy, as well as any time per the request of the patient or patient's agent; shall be communicated verbally, in person, unless the patient or patient's agent is not at the pharmacy or a specific communication barrier prohibits such verbal communication; and shall be documented either in the computer system and/or on the original hard copy of the prescription by the pharmacist that performed the counseling.

Responsible for directly overseeing the preparation and dispensing of all prescriptions received by the pharmacy, including supervision of certified pharmacy technicians during preparation of said prescriptions. Provide pharmaceutical care including patient drug regimen reviews, entering home medications in the electronic medical record (EMR), and providing drug information to the healthcare team and patients.

For a drug regimen review, a pharmacist shall, prior to or at the time of dispensing a prescription drug order, review the patient's medication record. Such review shall, at a minimum, identify: known allergies; rational therapy-contraindications; reasonable dose and route of administration; reasonable directions for use; duplication of therapy; drug-drug interactions; drug-food interactions; drug-disease interactions; adverse drug reactions; and proper utilization, including overutilization or underutilization. Upon identifying any clinically significant conditions, situations, or items listed above, the pharmacist shall take appropriate steps to avoid or resolve the problem including consultation with the prescribing practitioner. The pharmacist shall document such occurrences and make all necessary adjustments as authorized by the prescribing physician.

Responsible for facilitating departmental communication by demonstrating a professional, positive and helpful attitude in interactions with co-workers. Oversee drug inventories and insure that controlled substance inventories are completed quarterly, as required, and that a perpetual inventory is kept of all CII medications and all other controlled substances that require such inventory. Assist pharmacy technicians in maintaining adequate documentation, records, weekly inventory, and interactions with other health care professionals and patients. Maintain and generate patient reports and records using the current pharmacy computer system. Contribute to the effective operation of the pharmacy department, organize and prioritize work assignments, manage workflow, and assist the technician(s) with all aspects of the pharmacy operations. Maintain education, skill assessment testing and competency requirements for current job title and position. Requirements include providing a true and correct copy of current pharmacy license and receipts from required continuing education (ACPE accredited courses). Other duties as assigned.

QUALIFICATIONS:

Education: Graduate of an ACPE accredited School of Pharmacy. Licenses: Current State of Texas Pharmacy license Experience: Two years of pharmacy experience, retail pharmacy experience preferred, knowledge of medications involved with oncology and/or chemotherapy preferred.

SPECIAL EQUIPMENT REQUIREMENTS OR SKILLS REQUIRED:

Familiarity with telephone, copier, facsimile machine, computer equipment and software (Microsoft Word and Excel). Bilingual recommended
