Elm Manor Nursing and Rehab Center is seeking a licensed and compassionate Speech‑Language Pathologist (SLP) to join our dedicated in‑house rehabilitation team. As an integral part of our skilled nursing facility, you will provide evaluation, treatment, and care planning to patients recovering from illness, surgery, or injury. We offer a collaborative, patient-centered environment where your expertise makes a direct and lasting impact on the lives of those we serve. Part‑time or consistent PRN available Responsibilities: Evaluate and diagnose speech, language, communication, and swallowing disorders Develop and implement individualized treatment plans Collaborate with physicians, nursing staff, and rehab team to ensure quality care Document patient progress and maintain compliance with all regulatory guidelines Educate patients and families on therapy goals and techniques for carryover Participate in interdisciplinary team meetings and discharge planning Qualifications: Master’s degree in Speech‑Language Pathology from an accredited program Current New York SLP license (or eligibility) Certificate of Clinical Competence (CCC‑SLP) preferred Prior experience in a subacute/skilled nursing setting is a plus Strong communication, clinical, and organizational skills Commitment to patient‑centered care and outcomes #J-18808-Ljbffr Elm Manor Nursing And Rehabilitation
