Hospice Social Worker

A respected community hospital in Arkansas, just outside of Branson, Missouri, is seeking a Hospice Social Worker to join their Home Health & Hospice team.

Offering $24.45 - $33.00/hour + $5,000 Sign-On Bonus

Hospice Social Worker Opportunity:

• Full-Time, Permanent, Direct-Hire, Opportunity
  
• Monday - Friday, with rotating weekend on-call

Hospice Social Worker Responsibilities:

• Be a core member of the Hospice team and provide support to patient's and family's emotional, social, and economic needs relating to the impact of a terminal illness
  
• Plan the transition and transportation of patients from hospitals to a home or another facility, arrange services, collaborate with doctors, nurses, and insurers
  
• Plan and Lead interdisciplinary meetings, facilitate the referrals for community resources, and review critical access admissions and monitor patients with regularly scheduled admission visits
  

Hospice Social Worker Qualifications:

• Must have an Arkansas LCSW or LMSW or be eligible to obtain one
  
• Must have or be eligible to obtain BLS Certification
