Remote RN Telehealth LTSS Care Coordinator
Elevance Health
Elevance Health is seeking an RN Telehealth LTSS Service Coordinator to manage members' cases within licensure and develop care plans that optimize health across the care continuum. This role may supervise non-RN clinicians and coordinate across care teams. Ideal candidates hold an RN license with 3+ years in chronic illness care, and are eligible to work in a hybrid/remote capacity from the tri-state area (NY, NJ, CT). In-person training sessions may be required.
