Family Care Coordinator
Based in San Leandro, Building Futures is an Alameda County leader providing a safety net of services to help individuals and families rebuild safe and stable lives after homelessness and family violence. To accomplish this aim, the agency provides a continuum of care including two homeless shelters; a domestic violence safe house; a full spectrum of domestic violence outreach services; a transitional and permanent housing site; and rapid rehousing programs which serves target populations including military veterans, families reuniting after CPS involvement, survivors of domestic violence, and others.
The Family Care Coordinator provides front-end diversion, problem-solving, and housing stabilization services to individuals and families experiencing or at imminent risk of homelessness. The position works collaboratively with participants to identify safe alternatives to homelessness, resolve immediate housing crises, strengthen natural support systems, and connect individuals to community resources. Using a Housing First, trauma-informed, strengths-based, and participant-centered approach, the Family Care Coordinator assists participants in identifying realistic housing solutions while maintaining compliance with agency policies, funder requirements, and applicable federal, state, and local regulations.
Essential Functions:
- Conduct participant-centered problem-solving and diversion conversations to identify immediate housing options and alternatives to homelessness.
- Manage an assigned caseload and provide housing-focused case management, advocacy, follow-up services, and ongoing support to assist participants in achieving and maintaining housing stability.
- Maintain accurate daily logs, case notes, and files for all clients.
- Conduct housing search activities, including identifying available housing opportunities, assisting with housing applications, and coordinating lease-up activities.
- Facilitate housing and life skills workshops
- Collaborate with Street Outreach teams in searching for participants at encampments or meeting participants where they are located.
- Assess participants' housing crises, strengths, barriers, and available support networks.
- Assist participants in identifying safe alternatives to emergency shelter whenever appropriate.
- Develop individualized housing stabilization and problem-solving plans with participants.
- Provide mediation and conflict resolution support between participants, family members, landlords, or other members of a participant's support network, as appropriate.
- Connect participants with emergency financial assistance, mainstream benefits, employment resources, legal assistance, healthcare, behavioral health, childcare, transportation, and other community resources.
- Coordinate referrals to Coordinated Entry, emergency shelter, homelessness prevention, rapid rehousing, permanent housing, and other appropriate housing interventions.
- Maintain accurate, timely, and confidential documentation in HMIS or other agency-approved information systems.
- Monitor participant progress and provide short-term follow-up to evaluate housing stability and service outcomes.
- Collaborate with community partners, housing providers, landlords, government agencies, and multidisciplinary teams to improve participant access to housing resources.
- Participate in staff meetings, case conferencing, required trainings, and community collaboration activities.
- Deliver services using Housing First, trauma-informed, culturally responsive, strengths-based, and participant-centered practices.
- Perform other duties and responsibilities as assigned in support of Building Futures' mission and program operations.
Skills & Abilities:
- Proficient in Microsoft Office Suite (Word, Excel, Outlook, Teams) and agency-approved software applications.
- Strong interviewing, active listening, motivational interviewing, and problem-solving skills.
- Ability to build trusting, respectful relationships with individuals and families from diverse backgrounds.
- Ability to effectively de-escalate conflict and resolve complex participant situations.
- Strong verbal, written, and interpersonal communication skills.
- Ability to work independently while collaborating effectively with multidisciplinary teams.
- Strong organizational, time management, and documentation skills.
- Ability to maintain accurate, timely, and confidential participant records.
Minimum Qualifications:
- Equivalent combination of education, training, and relevant professional experience required.
- Minimum of two (2) years of experience providing homelessness prevention, housing problem-solving, diversion, case management, housing navigation, or related human services.
- Experience working with individuals and families experiencing or at risk of homelessness preferred.
- Knowledge of homelessness prevention strategies, housing resources, mainstream benefits, community services, and systems of care.
- Knowledge of Housing First principles, trauma-informed service delivery, motivational interviewing, and strengths-based practices.
- Knowledge of HMIS or similar client information systems preferred.
- Valid California Driver's License, reliable transportation, and proof of automobile insurance required.
Physical Requirements & Working Conditions:
- Perform work in office, community, field, and outdoor environments, including shelters, encampments, participant residences, and other community locations.
- Frequently walk, stand, sit, bend, kneel, squat, reach, climb stairs, and occasionally lift or carry materials weighing up to 25 pounds.
- Use hands and fingers to operate computers, mobile devices, office equipment, and other technology required to perform job duties.
- Communicate effectively in person, by telephone, and through electronic communication.
- Maintain accurate, timely, and confidential documentation using agency-approved information systems.
- Work may require occasional evenings, weekends, or holidays based on program or participant needs.
