Aged Home Care Coordinator

Hands and Feet provides services through local churches to help people in need, including food distribution, NDIS services and aged home care. We are building a professional team to provide superior care for the elderly across Australia. Aged Care Coordinator – full‑time role reporting to the GM and Aged Home Care Committee. The coordinator will oversee, develop and manage the Aged Home Care Service, ensuring quality, compliance and sustainability. Responsibilities Identify and collaborate with local churches to provide care for elderly community members. Induct and train new staff. Sign up new consumers directly or through sub‑contractor arrangements. Report against quality systems, aged care policies and to the Aged Home Care Committee. Oversee financial management to ensure service viability and consumer satisfaction. Maintain Aged Care Quality Standards within the service. Implement a robust staff education and training program. Monitor services provided and conduct assessments, preparing care plans with consumers. Monitor staff performance and culture. Review incidents, complaints and reporting of mandatory indicators and SIRS to Commission and Board. Implement internal and external audits across clinical and non‑clinical services. Qualifications Management experience and leadership skills. Experience working in the aged care industry. Clinical care qualifications (e.g., Bachelor of Nursing, Bachelor of Health Science Nursing) are beneficial. Must hold a current Police Check Clearance and a Working with Children Check clearance.
