Administer prescribed medications and treatments accurately and safely. Monitor and record patient symptoms, reactions, and progress. Collaborate effectively with physicians, interdisciplinary team members, and families to ensure optimal patient outcomes. Maintain accurate and timely clinical documentation. Provide a positive, safe and therapeutic environment. Required Qualifications Active and unencumbered LPN or RN license in the state of Oregon Graduated from an accredited nursing program (LPN or RN) Current CPR/BLS certification (or ability to obtain within 30 days of hire) Excellent clinical assessment and critical thinking skills Strong interpersonal, organizational, and documentation abilities Ability to work effectively both independently and as part of a multidisciplinary team Familiarity with electronic health records (EHR) Successfully complete ORCHARDS background screening Preferred Experience Previous experience in a long‑term care facility IV certification or demonstrated experience in advanced clinical procedures Knowledge of applicable state and federal regulations governing patient care Physical Requirements Standing and walking for extended periods during the shift The ability to safely lift and transfer up to 50 lbs. without assistance Requires physical stamina to handle emergencies, moving equipment, and performing other medical tasks Ability to use full range of motion in body movements including bending, squatting, kneeling, stooping and crouching, reaching over shoulder Pushing and pulling carts and wheelchairs up to 200 lbs.
