Nurse Practitioner Opportunity - Annual Wellness & Population Health Location: Apple Valley, CA - San Bernardino 50m, LA 90mEmployment Type: Full-Time, permanent position Position OverviewA growing healthcare organization serving the High Desert region of California is seeking a compassionate and experienced Nurse Practitioner to join its collaborative clinical team.This role focuses on Annual Wellness Visits, chronic disease management, patient education, and population health initiatives while supporting continuity of care and value-based healthcare outcomes. Position Highlights Conduct CMS-compliant Annual Wellness Visits Support HCC risk adjustment through accurate chronic condition documentation Perform medication reconciliation and patient health assessments Develop and manage individualized patient care plans Diagnose and manage acute and chronic medical conditions Collaborate closely with primary care providers and interdisciplinary teams Educate patients and families regarding wellness, prevention, and treatment plans Participate in quality improvement and compliance initiatives Strong focus on patient-centered, community-based care Responsibilities Perform comprehensive physical exams and health assessments Order, interpret, and manage diagnostic testing Prescribe medications and coordinate follow-up care Monitor patient outcomes and adjust treatment plans accordingly Maintain accurate EMR documentation Ensure compliance with HIPAA, OSHA, and infection control standards Support continuity of care across clinical settings Qualifications Active and unrestricted California RN license Current California Nurse Practitioner license Furnishing License required AANP certification required Current DEA registration American Heart Association certifications: BLS required ACLS required PALS as applicable Minimum one year of recent related clinical experience preferred Strong communication, clinical decision-making, and problem-solving skills EMR and Microsoft Office proficiency required Valid driver's license and auto insurance if travel between sites is required Compensation & Benefits Competitive compensation package Comprehensive benefits package Supportive and collaborative work culture Continuing education and professional development opportunities Opportunity to make a meaningful impact within the local community Organization OverviewThis physician-led healthcare organization is dedicated to delivering compassionate, patient-centered care throughout the High Desert region.
