Oak Street Health
Oak Street Health seeks a Community Health Worker to bridge patients, families and medical systems, removing barriers and promoting health literacy in the community.
You’ll engage patients through in-person and phone outreach, connecting them with local resources, while protecting privacy and complying with HIPAA guidelines. The role requires flexibility, strong communication, and travel within the community.
