Licensed Practical Nurse (LPN)

Full Time: 7pm-7:30am, 7am-7:30pm

Part Time: 3pm-11pm

As a Licensed Practical Nurse (LPN), you will play a vital role in providing high-quality care and support to our residents, helping them to recover and thrive in a rehabilitation setting.

About the Role:

As a Licensed Practical Nurse (LPN), you will work closely with our interdisciplinary team to deliver comprehensive patient care, focusing on the physical, emotional, and social needs of our residents. Your responsibilities will include:

• Providing direct patient care, including administering medications, dressing wounds, and performing vital signs
• Developing and implementing individualized care plans in collaboration with the healthcare team
• Assisting with daily living activities, such as bathing, dressing, and grooming
• Monitoring and reporting changes in patient condition to the healthcare team
• Documenting patient information accurately and thoroughly
• Maintaining a clean and safe environment for our residents

Requirements:

• Current LPN license in the state of Pennsylvania

• High school diploma or equivalent required

• 1-2 years of experience as an LPN preferred
