Registered Nurse – Home Health

Schedule: 8-hour shifts

Assignment Details:

• Contract Length: 14 weeks
• Guaranteed Hours: 40 per week

Requirements:

• Current state license as a Registered Nurse
• Homecare Homebase Software Experience
• OASIS Experience
• 1-2 years experience as a Registered Nurse in a clinical care setting
• 2+ years Home health experience
• CHPN certification (Preferred)
• Proof of current CPR certification
• Current driver's license
• Reliable transportation and insurance

Roles & Responsibilities:

• Assess patients in home health and hospice settings to identify their physical, psychosocial, and environmental needs.
• Develop, document, and evaluate the Plan of Care (POC) for quality and continuity of care.
• Educate patients and families on care, medication management, and self-care practices.
• Supervise and provide clinical direction to Home Health Aides and licensed nursing staff.
• Implement infection control measures to protect both staff and patients.
• Perform on-call responsibilities and provide support to patients and families as required.

Must maintain confidentiality and adhere to company policies regarding background checks and drug screens.
