Pay: $40.00 – $60.00 per visit

Position: Registered Nurse — Per Visit, Intermittent Skilled Home Health

Job description:

Procare Home Health is seeking a Registered Nurse who is currently working in home health performing patient visits and completing OASIS assessments, in the Fort Lauderdale area.

This is a per-visit position. It is not private duty nursing. There are no shifts and no hourly cases. Nurses are paid per completed visit and are expected to be able to open a case, complete the OASIS in the EMR, and submit documentation within agency timeframes without training on the fundamentals of the visit model.

Only nurses currently performing home health visits need apply.

Duties during patient visits vary with the plan of care and the patient:

Complete Start of Care, Resumption of Care, Recertification, and Discharge OASIS assessments and submit them within agency timeframes.

Assess and chart observations of the patient's condition at each visit.

Complete evaluation tasks, including medication review and vital signs.

Administer physician-prescribed medication.

Dress or redress wounds and assess healing progress.

Educate patients and families on home health care procedures and self-management.

Coordinate with physicians, physical therapists, occupational therapists, and others on the plan of care.

Keep the patient comfortable through palliative measures within the plan of care.

Recommend tools or devices that may improve the patient's quality of life.

Requirements:

Current, active Florida Registered Nurse license — required

Currently performing home health visits, with OASIS assessment experience — required

BLS/CPR certification — required

Reliable transportation, valid Florida driver license, and current auto insurance — required

Professional liability insurance

Ability to complete documentation electronically in the agency EMR
