Long-Term Care Pharmacist

The Long-Term Care Pharmacist will provide clinical pharmacy services to patients in assisted living and long-term care settings , with a focus on safe and effective medication management and quality patient care, as well as day-to-day pharmacy operations across multiple pharmacy locations throughout the Tampa Bay area. This position offers the opportunity to work closely with long-term care patients and healthcare teams, counsel patients, manage complex medication cases, and contribute to the development of innovative pharmacy services.

Unlike traditional retail pharmacy roles, this position provides the time and autonomy to focus on quality patient care, medication management, and meaningful relationships with patients and healthcare providers.

This is a full-time salaried position, typically working 40-45 hours per week .

This position provides pharmacist coverage across multiple pharmacy locations. The primary coverage area includes Tampa, Clearwater, and St. Petersburg, with occasional travel between pharmacy locations and to additional locations in Zephyrhills and Dade City as business needs require.

Key Responsibilities

• Dispense prescriptions and perform final verification
• Provide patient counseling and medication education
• Support the medication needs of long-term care patients and collaborate with their healthcare teams
• Oversee pharmacy workflow and support pharmacy staff
• Manage inventory and supply processes
• Resolve insurance and prior authorization challenges
• Collaborate with providers to improve patient outcomes
• Participate in community outreach and business development initiatives
• Support specialty pharmacy programs and patient access initiatives
• Provide pharmacist coverage at multiple pharmacy locations as needed
• Travel occasionally between pharmacy locations, including Tampa, Clearwater, St. Petersburg, Zephyrhills, and Dade City
• Ensure compliance with state and federal pharmacy regulations
