This role involves advocacy, service linkage, and monitoring to ensure continuity of care and prevent service gaps.Persons with lived experience in mental illness, developmental disabilities, or substance use disorders are encouraged to apply.Key ResponsibilitiesDevelop and implement person-centered service plansLink individuals to medical, social, educational, and therapeutic resourcesCoordinate and monitor services to achieve successful outcomesProvide advocacy and support to reduce barriers to careParticipate in emergency on-call rotationQualificationsBachelor’s degree in a human services fieldMinimum one year of experience in mental healthCertification as QMHP, QIDP, and/or CMHP, or Michigan licensure (limited or full)Strong organizational, communication, and problem-solving skillsAbility to work flexible hours and travel within service areaValid Michigan driver’s license, vehicle insurance, and satisfactory driving recordAbout WellvanceWellvance provides state-funded mental health services to residents of Iosco, Ogemaw, and Oscoda counties.
