Psychiatric Nurse Practitioner, Outpatient Group Practice - Illinois

Perform complete initial psychiatric evaluation on patient to determine diagnosis and develop plan of care including medication prescription, if indicated

Revise, prescribe, and implement therapeutic regimen using an interdisciplinary approach

Monitor patient progress and provide collaborative treatment and communication with the treatment team

Provide timely documentation of each encounter with a patient

Ensure patient|family education regarding clinical condition and relevant aspects of care

Must be able to work in collaboration with treatment team members to ensure quality of care and consideration of services
