Nurse Practitioner OpportunityLocation: Tucson, AZSign-On and Relocation Bonus OfferedImpresiv Health is seeking a Nurse Practitioner to join an interdisciplinary care team providing comprehensive medical care and chronic disease management to a focused panel of patients living with chronic kidney disease (CKD) and other complex conditions. This role focuses on preventive care, chronic disease management, patient education, and care coordination through a value-based, relationship-centered approach. The Nurse Practitioner will work closely with physicians, Social Workers, Care Navigators, specialists, and community resources to ensure care is coordinated, connected, and centered around the whole person. Patients are seen in their homes, virtually, or by phone, providing the opportunity to build meaningful relationships and gain a deeper understanding of each patient's unique health needs.Schedule and Work Model:Monday through Friday, 8:00 AM to 5:00 PM, with flexibility based on patient needsNo routine weekends or overnight shiftsLimited after-hours callsApproximately 5 to 6 patient visits per dayCombination of in-home visits, telehealth, and care coordinationDaily collaboration with patients and interdisciplinary care team members to coordinate comprehensive, high-quality careWhat You Will Do:Manage a small panel of patients with CKD and other complex chronic conditions alongside an interdisciplinary care teamConduct comprehensive health assessments, medical histories, physical examinations, and ongoing clinical evaluationsDiagnose, treat, and manage acute and chronic conditions within scope of practiceOrder, interpret, and review laboratory and diagnostic testingPrescribe and manage medications in accordance with state regulationsDevelop individualized care plans addressing each patient's medical, behavioral, and social needsEducate patients and caregivers on disease prevention, medication adherence, lifestyle changes, and treatment plansCoordinate care across primary care providers, specialists, hospitals, skilled nursing facilities, home health, hospice, and community organizationsParticipate in regular interdisciplinary care team huddlesMonitor patient progress and adjust care plans to improve outcomes and reduce avoidable hospitalizationsDocument clinical encounters accurately and on timeSupport quality improvement initiatives and evidence-based practiceMaintain patient confidentiality and comply with company policies and regulatory requirementsProvide after-hours telephonic support as neededYou Will Be Successful If:You are comfortable working independently while collaborating closely within a teamYou are comfortable delivering care both in person and via telehealthYou have a genuine passion for relationship-centered, value-based care for patients with chronic illnessWhat You Will Bring:Active Nurse Practitioner or Physician Assistant license in Arizona or FloridaBoard certificationAbility to meet clinician credentialing requirements, including a background checkValid driver's license, reliable transportation, and the ability to travel to patient homesStrong clinical assessment, communication, and critical-thinking skillsComfort working independently while collaborating closely within a teamComfort delivering care both in person and via telehealthA genuine passion for relationship-centered, value-based care for patients with chronic illnessPreferred Experience:Experience in primary care, internal medicine, geriatrics, home-based care, urgent care, or value-based careExperience caring for patients with multiple chronic conditionsAbout Impresiv Health:Impresiv Health is a healthcare consulting partner specializing in clinical & operations management, enterprise project management, professional services, and software consulting services.
