Nurse Practitioner

We are actively seeking a Nurse Practitioner (FNP or PMHNP) or Physician Assistant (PA) to support our Assertive Community Treatment (ACT) Team and Outpatient Program (OPT) in Lumberton, NC. The Assertive Community Treatment (ACT) team consists of a community-based group of medical, behavioral health, and rehabilitation professionals who use a team approach to meet the needs of a patient with severe and persistent mental illness. The ACT team assists a patient in advancing toward personal goals with a focus on enhancing community integration. As an ACT Team psychiatric care provider (NP or PA) you are integrated on the team and provide direct services to patients. Location: Lumberton, NC Compensation: $130,615 - $144,535 Build Your Purpose. Position Description: Sees clients for the assessment and treatment of the client’s symptoms and response to medication including side effects; Frequency will vary for each client, with the majority seen within 4 to 6 weeks of last appointment, with many of the contacts being in the community. Collaborates with the ACT team leader in sharing overall clinical responsibility for monitoring beneficiary treatment and clinical supervision to the team; Actively collaborates with nurses to develop and implement medication administration policies and procedures as well as oversee the medical care of clients that include regular screenings for medical conditions and assessment of wellness and health management; Educates non-medical team members on psychiatric and non-psychiatric medications, their side effects, and health-related conditions; provides diagnostic and medication education to beneficiary, with medication decisions based on shared decision making; Participates in daily team meetings and treatment planning meetings; Provides psychiatric back-up to the program after-hours and weekends. Maintain patient confidentiality according to HIPAA regulations Consults with physicians on cases that are beyond the scope of qualification of PMHNP. Communicates with patients and engages in follow‑up in‑between appointments, as necessary. Maintain timely patient documentation through our EHR system Performs similar or related duties as assigned or required. Must be open to continuing education. Position Qualifications: Licenses: Nurse Practitioner (FNP or PMHNP) with a current NC license that meets the requirements as specified in 21NCAC 36.0800 with at least three years full‑time experience treating individuals with SPMI* Current/Active DEA license/certification Be an approved Medicaid biller or eligible to become an approved Medicaid biller. Proven ability to communicate effectively with strong verbal skills Excellent interpersonal and customer service skills Must have a valid driver’s license and minimum state required liability auto insurance; Local travel is required Proficiency with electronic records management * SPMI - Severe and Persistent Mental Illness is defined as a mental disorder in adults (18+) that severely impairs daily functioning in areas like self‑care, relationships, or employment, requiring long‑term support, or is evidenced by receiving SSI/SSDI due to mental illness.
