PT - Physical Therapist

Outpatient Physical Therapist

This is a 13-week contract position.

5 days a week, 8 hours per day.

The Outpatient Physical Therapist will evaluate all assigned patients and develop, implement, and modify treatment plans in accordance with the patient's needs, goals, and physician direction. They are responsible for the delegation and direction of all discipline-specific therapy services for those assigned patients while ensuring the design of functional treatment programs that effectively facilitate a return to community activities. The role also involves educating patients, families, students, coworkers, and self, while maintaining professional standards and upholding expectations of compliance with the organizational Code of Conduct.

Responsibilities:

• Complete comprehensive evaluations of assigned patients.
• Utilize clinical reasoning to develop treatment plans and goals aimed at progressing patients towards discharge.
• Document medical records clearly, accurately, and promptly in compliance with regulatory guidelines.
• Participate in student programs as assigned, providing a positive learning environment.
• Create an individualized professional development plan and pursue opportunities for professional growth.
• Adhere to State Practice Act and maintain professional licensure in good standing.
• Engage in performance improvement initiatives to enhance service quality.

Required Experience / Certifications / Licensure:

• Degree in Physical Therapy from an accredited school.
• Minimum of 1 year of outpatient clinical experience.
• Current CPR certification.
• Active and applicable state licensure for practice.
