This position offers a unique opportunity to enhance your career while being surrounded by breathtaking mountain views and numerous recreational activities.Job OverviewPosition: Case ManagerDepartment: RehabilitationShift: DaysWeekly Guaranteed Hours: 35Weekend Hours: 1-5Call Requirements: NoPosition ResponsibilitiesManage a caseload to ensure comprehensive patient care.Collaborate with a multidisciplinary team to develop and implement individualized treatment plans.Conduct assessments and evaluations to create effective care strategies.Facilitate communication between patients, families, and healthcare providers.Monitor patient progress and adjust care plans as needed.QualificationsRequired Education: RN LicenseYears of Experience: 1+ year in Case ManagementMust Have Skills: Experience in Case ManagementPreferred Skills: Familiarity with EPIC electronic medical recordsUnit and Patient InformationNumber of Beds: 26Patient Population: Inpatient Rehabilitation Facility (IRF) including cases such as Stroke, Traumatic Brain Injury (TBI), hip fractures, trauma, acute renal failure, wound care, and amputations.Patient Ratios: ½ census caseloadEquipment: EMR: EPICWork EnvironmentEnjoy a collaborative and flexible work environment where you can thrive professionally.
