Minimum Requirements
Master's or Doctoral Degree in Physical Therapy from an accredited program.
Active Texas Physical Therapist license required prior to start.
BLS certification from AHA or ARC may be required.
Home health and HCHB experience required.
Experience with home health documentation: SOCs, ROCs, recertifications/reassessments, and plan of care development.
Experience supervising PTAs.
Preferred: orthopedics and neuro experience.
Assignment Details
Facility Type: Home Health.
Shift Name: Days.
Shift Details: Monday–Friday, 08:00–16:00.
Weekly Hours: 40.
Productivity Expectation: 30 points.
Coverage Areas: Hawkins, Mineola, Lindale (local travel between patient visits required). Mileage reimbursed between visits from first to last patient.
Primary responsibilities: patient assessment, complete plan of care for home health, SOCs, ROCs, recerts/reassessments, supervise PTA.
Dress code: scrubs in black, gray, or purple.
