Physical Therapist – Physical Therapy

Schedule: 8-hour shift

Assignment Details:

• Contract Length: 13 weeks
• Guaranteed Hours: 40

Requirements:

• Current state license as a physical therapist
• Proof of current CPR certification
• Current driver's license with reliable transportation and insurance
• 1-2 years of experience as a physical therapist in an acute care or rehabilitation setting
• Previous home care experience (preferred)
• Bachelor of Science in Physical Therapy from an American Physical Therapy Association approved program

Roles & Responsibilities:

• Perform clinical assessments to identify patient needs and determine appropriateness for physical therapy.
• Document and prepare a Plan of Care (POC), progress notes, and other clinical records based on ongoing assessment data.
• Plan and provide treatment for neuromuscular, neurological, and orthopedic disorders based on medical referrals.
• Instruct patients and families in transfer techniques and the use of therapeutic equipment to ensure safety and proper usage.
• Provide patient and family education regarding therapeutic procedures, tailored to the patient's age and condition.
• Work collaboratively with other team members to ensure consistent and appropriate patient care.
• Apply infection control and universal precautions in all patient care activities.
