Home Care Coordinator – Bilingual Preferred Home Care of New York, a rapidly growing Licensed Home Care Agency serving the NY Metropolitan area is seeking an experienced bilingual Home Care Coordinator for our Brooklyn Corporate Office. POSITION SUMMARY: A Home Care Coordinator executes and supports each client’s home care solution. This includes maintenance of ongoing communication and coordination with the referral, family members, client, healthcare providers and intake sources for all clients. The Home Care Coordinator shall plan for all home healthcare service coverage from therapy, nursing, caregivers, and companions, while ensuring that the patient/family are receiving excellent customer service.
