All members of our organization have a responsibility to uphold these values.Santé is seeking a School Intervention Specialist they will be responsible for the assessment and in home treatment for clients in the community. This individual provides direct care, documents all visits and interactions associated with the case, is the onsite resource for any relevant clinical information and the liaison for collaboration with other involved parties including community providers.What You'll Do:Perform an environmental and safety assessment upon entry to the community or residential setting where visit is to take placePerform assessment for SI and develop safety plan if necessaryPerform an assessment of client and diagnostic impressionDevelop a treatment plan collaboratively with client, family, guidance counselor and out patient provider based on identified client needsCan effectively utilize crisis intervention skills when necessaryCan build rapport with a wide variety of clients and work from a culturally competent perspectiveCan identify need for medical and/or substance abuse treatment and make appropriate referralsHas adequate knowledge of community resources and provides appropriate referrals to clients both mental health and non-mental healthCan implement brief therapeutic strategies that are effective in reducing client's mental health symptomsWork with families and involved persons to de-escalate the situation, make a crisis plan, and enlist cooperation with recommended treatmentSet short-term and measurable goalsAble to demonstrate the capacity to work with a wide range of clients; child and adolescent, adult, dual diagnosis and geriatricComplete all documentation in a timely manner using EMRParticipate in all required trainingsAbide by all federal, state and local confidentiality regulationsComply with all mandatory reporting, including duty to warn and child abuse/neglect referralsComply with all of programs contractual and operational guidelines as outlined by your managerCheck work e-mail according to agency protocolOther duties as assignedWhat We Require:Bachelors Degree in Counseling, Social Work, or a related field of study.
