Nurse Case Manager

Role: Nurse Case Manager supporting youth with emotional, psychological, and physical needs in a residential setting. Key Responsibilities Provide medical case management, screening, education, and counseling services. Manage medications and coordinate with community healthcare providers. Participate in a rotating on-call schedule, primarily via phone consultations. Qualifications & Skills Bachelor's or Associate of Science in Nursing from an accredited program. Current Illinois RN licensure and valid driver's license. Preferred experience in psychiatric care and working with children/adolescents. Strong organizational, communication, and clinical skills. Key Duties & Focus: Supporting youth in need through clinical care, community networking, and accessible on-call support, ensuring holistic well-being. #J-18808-Ljbffr
