Job Summary:
We are seeking a compassionate and dedicated Licensed Practical Nurse (LPN) to join our team at Onyx Care of Winnfield. Responsibilities:
Provide skilled nursing care to patients in a post-acute rehabilitation setting
Deliver individualized care plans tailored to each patient's unique needs and recovery goals
Administer medications, treatments, and therapies as prescribed by physicians and other healthcare professionals
Monitor patients' health conditions and report any changes or concerns to the healthcare team
Collaborate with other healthcare professionals to ensure seamless patient care and support
Maintain accurate and detailed patient records and documentation
