Hybrid Nephrology RN Case Manager - Field Care Lead

DaVita Integrated Kidney Care (DaVita IKC) is seeking an RN Case Manager to coordinate care for ESRD and CKD patients across the Tacoma area. This hybrid, field-based role requires a dedicated home office and 3–4 days of travel each week to visit patients within the assigned territory. You will collaborate with patients, families, physicians, providers and care teams to deliver high-quality, cost-effective care, minimize fragmentation, and support transitions through the care continuum. #J-18808-Ljbffr DaVit
