SouthernCare, a Gentiva company, is seeking a Registered Nurse Admissions to join our compassionate hospice team in Tuscaloosa, AL. In this role, you’ll conduct comprehensive hospice admissions, complete assessments, and educate patients and families on care options and services. You will coordinate with an interdisciplinary team to develop and implement individualized care plans focused on comfort, dignity, and quality of life. The RN Admissions nurse ensures timely documentation, regulatory compliance, and smooth transitions of care while providing empathetic support, advocacy, and clear communication to patients, families, and providers.
