Position Summary The primary purpose of your job is to return patients to optimal physical function in a SNF setting. You will facilitate achievement of patient, physician, and therapy goals. Instruct the patient in maintaining achieved level of function after discharge through education, independent exercise program, and appropriate activity modification. Responsibilities Facilitate achievement of patient, physician, and therapy goals in a SNF setting. Instruct patients in maintaining achieved level of function after discharge via education, independent exercise programs, and activity modification. Education/Qualifications Master’s Degree in Speech-Language Pathology from an accredited college or university and an active, unencumbered license from the state of practice. Basic computer skills and ability to navigate service delivery software and electronic health record software.
